Introduction

Following a referral from the Local Authority, we were given a very short timeframe to establish a fully operational support package for a 17-year-old service user transitioning from a residential care setting.

Despite the short timeframe, our experienced bookings and operations team worked closely with commissioners to carefully match experienced support workers to the individual's needs and preferences. Alongside sourcing the right team, we ensured that all risk assessments and staff briefings were completed before the package commenced.

By taking a proactive and collaborative approach, we successfully delivered a safe, person-centred support package within the required timescales, providing reassurance to both the Local Authority and wider care team.

Care Package Overview

The service user had experienced significant childhood trauma, including family breakdown and substance misuse within the family home. Having lived in residential care for over two years, they required a tailored support package focused on promoting independence, improving wellbeing and encouraging positive engagement with education and the local community.

The the package began, the young person rarely left their room, struggled with personal hygiene and frequently disengaged from education. These complex needs required consistent support, positive relationship building and responsive decision making from our staff.

Within the first month, significant progress had been made and the service user became more confident travelling via public transport, established healthier daily routines and began engaging proactively with education and specialist drug and alcohol services.

Going Forward

Although challenges remain, the progress made throughout the support package has created a strong foundation for continued success.

Our team continues to work closely with the young person, Local Authority, social worker and wider multidisciplinary team to provide consistent, tailored support that adapts as their needs evolve.

Having relocated to another county within the North East, this has presented challenges with moving away from friends and family. Through the dedication of our staff, the service user has been able to remain connected with loved ones, participate more regularly in community activities and continue developing the confidence to travel.

As support continues, our primary focus remains on promoting independence, strengthening life skills and helping the service user achieve their long-term goals.
